OFA Eye Exams

Additional Information - Please Read: The Canine Eye Registration Foundation (CERF) was the only registry for screening eye examinations until recently when a second registry was established through the Orthopedic Foundation for Animals (OFA). The eye examination performed is the same for either registry as are the criteria for pass or fail, but the forms to be completed and submitted are different. The American College of Veterinary Ophthalmologists (ACVO) has endorsed the OFA registry in preference to CERF. Therefore, most ophthalmologists will provide OFA registry forms unless the pet owner specifically requests a CERF form. Both forms will be available at the eye clinic, but there will be a $2.00 surcharge for each CERF form requested. The cost of the OFA form is included in the examination fee. Date/Time: Friday, May 10, @ 1:30pm - 4pm^{* *}Allow 20 min. for eye drops to dilate eyes OFA approved veterinarian:  Keith Collins, DVM, MS, DAVCO Minimum number of exams necessary to hold clinic: 30 dogs, open to all breeds with Belgian Sheepdogs given priority. Cost: $30.00/dog Reservations required. A waiting list will be kept after the first 30 reservations. A reservation guarantees a spot in the clinic but exam time is first come, first serve based on a sign-in sheet the day of the clinic. You may not sign in friends. Be sure to have each dog’s AKC number at time of exam. For each reservation, include the following information: Your name Your mailing address Your e-mail address Dog’s call name Dog’s registered name Breed of dog Send reservation and check made out to BSCA to: Susan Spinhirne 26804 South Beverly Drive Monee IL  60449 NOTE: Clinic will be canceled if 30 reservations not received by April 10. All checks will be returned in the event of cancellation. If you cancel your reservation(s) before April 10, you will receive a refund and the next person on the waiting list will be notified.
